Semaglutide (trade names Ozempic®, Wegovy®, Rybelsus®) is a GLP-1 receptor agonist and is among the best-researched peptides in its class. It was originally developed to treat type 2 diabetes and is now also approved for obesity therapy due to its pronounced weight-reducing effect.
Mechanism of action of semaglutide
- Insulin secretion: Semaglutide stimulates glucose-dependent insulin secretion from the beta cells of the pancreas.
- Glucagon inhibition: It suppresses glucagon release, which further stabilizes blood sugar.
- Gastric emptying: It slows down gastric emptying and thus promotes a longer-lasting feeling of satiety.
- CNS effects: It acts on hunger and satiety centers in the hypothalamus and sustainably reduces calorie intake.
Semaglutide has a long half-life of approximately 1 week, which allows for weekly dosing intervals.
Clinical studies on semaglutide
The STEP 1 program (Novo Nordisk, 2021, published in New England Journal of MedicineThe study investigated semaglutide 2.4 mg over 68 weeks in obese adults. The result: an average body weight reduction of 14,9 % compared to 2.4 % in the placebo group.

Semaglutide is the longest-available and therefore best-documented peptide in this class. Tirzepatide and retatrutide achieve greater reductions in studies, but are newer and have less long-term data available.
