Tirzepatide (trade name Mounjaro®/Zepbound®, Eli Lilly) is a dual GIP/GLP-1 agonist. The peptide simultaneously activates the receptors for glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) – a class of drugs known as „twincretin“.
Mechanism of action of tirzepatide
- GLP-1 agonism: Stimulation of insulin secretion, inhibition of glucagon release, delayed gastric emptying. Acts as an appetite suppressant and blood sugar regulator.
- GIP agonism: It enhances the insulinotropic response, promotes fat oxidation, and improves insulin sensitivity in adipose tissue and the liver. Simultaneously, undesirable GLP-1 effects (e.g., nausea) are mitigated by the GIP component.
The combination of both pathways of action creates a synergistic effect that goes beyond that of pure GLP-1 monotherapy.
Clinical studies on tirzepatide
The SURMOUNT-1 program (Eli Lilly, 2022, published in New England Journal of MedicineThe study investigated tirzepatide over 72 weeks in obese individuals. The result: an average body weight reduction of up to... 22,5 % in the highest dosage group (15 mg) – significantly more than with semaglutide in comparable studies.
Tirzepatide vs. Semaglutide vs. Retatrutide
| peptide | agonism | Average reduction (study) | status |
|---|---|---|---|
| Semaglutide | GLP-1 | ~15 % (STEP-1) | Approved |
| Tirzepid | GLP-1, GIP | ~22.5 % (SURMOUNT-1) | Approved |
| Retatrutide | GLP-1, GIP, Glucagon | ~28.3 % (TRIUMPH-1) | Phase 3 |
